QEI Test Only Application
If you are interested in taking the QEI exam without attending the training course, you should submit the application below. Please be sure to provide copies of any certificates (including NEIEP, CAT and CET), licenses, degrees or other documentation to support your education and experience. Also include copies of any previous or current QEI certifications. Failure to provide this documentation may delay processing of your application.
Be sure to indicate the preferred testing location on your application. Tests may be taken at the Tumwater Office in WA or at any site where a test is being given following a QEI training course.
Payment must accompany application at the time of submittal.
